    RISE Community Health Centre (CHC) is the newest department of Collingwood Neighbourhood House. RISE is a community-based CHC that is part of the Primary Care Network strategy and funded by the Ministry of Health. RISE supports the health and well-being of Renfrew-Collingwood residents who are facing barriers to achieving health by offering community-based primary care and health promotion services. Populations of focus include Newcomers, LGBTQ2S+, Indigenous, Isolated Seniors, Youth, people experiencing Homelessness, those working in the Sex Trade, and those experiencing Mental Health and Substance Use issues. Services are delivered by a diverse interdisciplinary team, working in partnership with other Collingwood Neighbourhood House departments and partnering agencies.

    JOB OVERVIEW

    We are seeking a Physiotherapist to join the RISE Community Health Centre Team at CNH. The position provides individual physiotherapy treatment to RISE CHC Primary Care Clients and group physio-related health workshops to the broader Renfrew-Collingwood Community. The Physiotherapist will work as part of an interdisciplinary primary health care team, in collaboration with Nurse Practitioners, Physicians, Registered Nurses, Dietitians, Counsellors, Social Workers, Community health workers, medical office assistants and more. Referrals to physiotherapy are made in-house and collaboration of care is well supported amongst all team members. The physiotherapist will utilize their skill-set by providing manual therapy, exercise teaching, home program prescription, and education. 

    SKILLS & QUALIFICATIONS

    • Licensed with the College of Physical Therapists of British Columbia
    • Preferred two years or more of experience providing physiotherapy as part of an interdisciplinary team in a community setting
    • Training and demonstrated ability to provide physiotherapy assessment and treatment related across the lifespan, to clients with a variety of orthopedic conditions including but not limited to sprains/strains, tendinopathies, chronic pain syndrome, posture correction, concussion therapy and vestibular rehabilitation
    • Demonstrated ability to provide physio-related health teaching in a group setting
    • An egalitarian philosophy and non-judgmental attitude (i.e. gender, culture, age, race, sexual orientation, language, ability and more)
    • Ability to provide services through outreach
    • Previous experience providing culturally safe, trauma-informed care to marginalized populations

    JOB OVERVIEW  

    The light housekeeping and companionship program provides affordable, regularly scheduled cleaning services to seniors in the Renfrew-Collingwood community and offers a friendly visit with each appointment to socially isolated clients to increase connection and offset the adverse effects of loneliness. We are looking for two part-time, permanent housekeeper/friendly visitors to help fill the growing demand for light housekeeping and social connections.

    SKILLS, QUALIFICATIONS & EXPERIENCE

    • Good communication, interpersonal and problem-solving skills
    • Patience and care for seniors’ needs
    • Experience with general housecleaning
    • Ability to use digital devices to update calendar and fill out timesheets
    • Speaks English fluently (Cantonese, Mandarin or other additional languages would be an asset)
    • Excellent time management skills

    2. Health and wellbeing:

    JOB OVERVIEW

    The Director of RISE CHC is a key member of the Senior Leadership Team at Collingwood Neighbourhood House (CNH), reporting directly to the Executive Director. This position provides strategic oversight for RISE CHC and direct support for the RISE CHC and RISE CHC Pharmacy managers and the RISE CHC clinical lead.

    This role brings together deep expertise in health care systems, public health principles, primary care and health-promotion service delivery, the CHC model and principles, and organizational development to lead and support a diverse workforce in a non-profit setting. The Director is responsible for the effective management and integration of the RISE CHC Primary Care Clinic and Pharmacy and works collaboratively across department and programs to support health promotion in the Renfrew-Collingwood Neighbourhood. 

    The Director models and promotes core leadership competencies including strategic thinking, change management, emotional intelligence, decision making, communication, team building, diversity and inclusion, ethical stewardship, innovation, and creativity. By fostering a culture of accountability, well-being, and collaboration, the Director contributes to the long-term sustainability and integrity of CNH and the community it serves.

    RESPONSIBILITIES

    (a) Strategic and Organizational Leadership

    • Provide strategic RISE CHC leadership aligned with CNH’s mission, values, and community objectives
    • Actively participate in the Senior Leadership Team, contributing to long-term planning and cross-organizational initiatives
    • Represent RISE CHC in local and provincial initiatives such as with the local Primary Care Network (PCN3), Ministry of Health Strategy Funded Community Health Centres, the Canadian and BC Association of Community Health Centres, local health authorities, and others
    • Champion a respectful, inclusive, and service-oriented culture across the RISE CHC department
    • Build and maintain strong working relationships with internal teams, external partners, funders, and government representatives

    (c) Human Resources Management and Organizational Development Acumen

    • Support RISE CHC human resources and organizational health, by fostering a respectful, inclusive, and effective workplace culture aligned with CNH’s values and operational priorities
    • In collaboration with CNH HR and Finance, and RISE CHC managers, support RISE CHC recruitment, retention, onboarding, performance development and management, professional development, student placements, succession planning, and workforce planning
    • Support the HR team in applying employment legislation, interpreting and implementing the Physician Master Agreement and other relevant healthcare agreements and standards while upholding CNH’s internal policies and procedures
    • In collaboration with CNH HR and Finance department, provides leadership in RISE CHC labour relations, conflict resolution, and employee relations
    • In collaboration with CNH HR and Finance department ensure compliance with all relevant legal and regulatory frameworks, including the BC Employment Standards Act, WorkSafeBC, human rights legislation, health care specific regulations and scopes of practice, and privacy laws
    • In collaboration with CNH HR and Finance Department, develop and manage labour contracts including for Physicians, clinical supervisor, and others
    • Promote a healthy, inclusive, and psychologically safe workplace by integrating occupational health and safety (OHS) principles and organizational behaviour best practices
    • Utilize HR metrics and data analysis to inform decision-making, identify trends, and proactively address workforce needs
    • Advise on RISE CHC positions role profiles, compensation, benefits, and total rewards frameworks in collaboration with the Director of HR and Finance, ensuring they are equitable, sustainable, and supportive of employee well-being
    • Champion employee engagement and organizational development initiatives that build capacity, strengthen team dynamics, and enhance individual and collective resilience
    • Directly supervise RISE CHC Manager, Clinical Lead, Pharmacy Manager and contractors as relevant; and support Managers and clinical lead in supervising an additional 18-20 FTEs/40–50 clinical and administrative staff and contractors
    • Ensure annual renewal of malpractice insurance and that RISE CHC staff are working within their regulated scope of practice and within limits of RISE CHC insurance policies

    (d) Contract Negotiation and Compliance

    • Lead the negotiation and administration of funding contracts and service agreements within the RISE CHC department, ensuring they align with CNH’s operational capacity and mission
    • Ensure RISE CHC services are being delivered in compliance with RISE CHC funding contracts
    • Submit regular financial reports to the funder with the support of CNH accounting
    • Submit regular statistical and qualitative reports to funders

    (e) Mentorship and Team Support

    • Provide direct supervision, coaching, and performance oversight to the RISE CHC management team
    • Promote a learning-focused, team-based environment that encourages team members participation in decision-making and continuous improvement
    • Promote continuing education opportunities for staff

    (f) Administration and Grant Writing

    • Apply for and support RISE CHC staff to apply for grants and funding. Oversee all RISE CHC funding and ensure funding deliverables are achieved and reported on following grant requirements within the project timeframes
    • Perform routine administrative duties, including approving cheque requisitions, payroll review, and time-tracking approval as required based upon financial policies and procedures
    • Perform other administrative tasks and responsibilities as required

      (g) Evaluation and Quality Improvement

    • Develop and review RISE CHC evaluation framework and implementation, in collaboration with internal and external evaluators
    • Monitor and evaluate services to measure successes and identify opportunities for improvement
    • Conduct ongoing needs assessment and evaluation of all RISE CHC programs and Renfrew-Collingwood health needs, together with Clinical Lead, Clinic Manager and Pharmacy manager
    • Create evidence-based quality enhancement goals, co-create a strategy and ensure progress to meeting those goals
    • Provide quality program planning, implementation and evaluation by ensuring capital, human, financial, and communication resources are sufficient
    • Ensure that staff are aware of and meet all quality and safety standards
    • Work with RISE CHC Advisory Committee and staff to ensure that community needs are understood and RISE CHC’s objectives are fulfilled through services, programs, partnerships, and community outreach

    (h) Service Planning and Delivery

    • In collaboration with RISE CHC staff, establish and implement policies and procedures for all functions and for the day-to-day operations of RISE CHC
    • Liaise, cooperate and collaborate with CNH staff, community agencies, public partners, health care partners, and residents in the identification, provision, coordination, and evaluation of health care and health promotion programs and services in Renfrew-Collingwood
    • Support planning, development and implementation of health care and health promotion services to meet community needs and funding requirements
    • Ensure RISE CHC services are delivered according to CNH mission vision and values, CHC Principles, and RISE CHC shared purpose

    (i) Facilities Coordination

    • In coordination with CNH Operations Department, ensure that 5198 Joyce Street has appropriate insurance coverage (Commercial General Liability)
    • In coordination with the RISE CHC managers, and landlord (VCH), ensure the 5198 Joyce Street is safe, sanitary, well-maintained and that equipment is in good working order, purchased and repaired as needed
    • In coordination with IT and the RISE CHC Managers, ensure the data systems are secure and ensure IT needs are met
    • In coordination with CNH Operations and the RISE CHC management team, oversee telecommunications contracts and services for 5198 Joyce Street
    • In coordination with CNH Operations and the RISE CHC management team manage the various facilities contracts of the clinic (i.e. janitorial, security, telecommunication, computer, etc.)
    • Support RISE CHC managers to direct/oversee all trades people for the RISE CHC Building
    • Respond to RISE CHC building alarms and emergencies
    • In coordination with CNH operations, ensure 5198 Joyce Street has appropriate permits in place at all time
    • Negotiate the lease for 5198 Joyce Street or develop and execute plans for future spaces/renovations
    • In collaboration with the RISE CHC/Evergreen Occupational Health and Safety Committee, develop and implement occupational health and safety and infection control policies and procedures for 5198 Joyce Street and ensure adherence to policies/procedures

    SKILLS AND QUALIFICATIONS

    • Education
      • Post-secondary degree in a health-related field (Master’s preferred) with demonstrated expertise in the BC health care system and social determinants of health
      • Current registration/membership with a recognized healthcare regulatory body is an asset
    • Leadership Experience
      • Minimum 6 years of progressive leadership experience in a health care, preferably in a non- profit or community-based organization, with experience supervising an interdisciplinary health care team
    • Financial Expertise
      • Proven ability to manage a budget of at least $3 million annually, including budget preparation, analysis, decision-making, and reporting
      • Proven ability to effectively manage a social enterprise to create financial surplus
    • Human Resources and OHS Knowledge
      • Proficiency in health care human resources and organizational development
      • Proven ability to work with health care professionals in an interdisciplinary health care setting
      • Comprehensive understanding of the clinical and non-clinical positions at RISE CHC, including scope of practice, credentials, and relevant wage grids
      • Demonstrated ability to interpret and implement employment and OHS legislation into practical workplace policies and systems

    Collingwood Neighbourhood House (CNH) is seeking a Canadian IT consultant or firm to support a comprehensive review and improvement of our technology systems. This work will include assessing our infrastructure, auditing current IT practices, enhancing cybersecurity, improving data collection and digital safety, and supporting staff training on new tools and systems.

    Background 

    CNH is a non-profit organization that provides community programs and services to residents of Renfrew-Collingwood in East Vancouver. Our mandate is to promote the well-being of the Renfrew-Collingwood community by providing leadership and working collaboratively with individuals, families, agencies, and other groups to develop and support inclusive, innovative, sustainable initiatives and services that respond to the community’s social, educational, economic, health, cultural and recreational needs. We offer programs and services in areas such as childcare, healthcare, recreation, seniors, youth, newcomers, arts and culture, food justice and community engagement.  

    As our organization has grown—most notably with the recent addition of a health care centre and a continually expanding range of programs—our technology needs have become more diverse and complex. This growth presents an important opportunity to develop a comprehensive IT plan that aligns with our current operations and supports future needs. With a wide range of users, systems, and service areas, we are seeking an IT contractor to help us assess our existing infrastructure and practices, identify gaps, and build a more coordinated, efficient, and responsive approach to technology across the organization. 

    CNH employs approximately 68 full-time and 133 part-time staff and more than 200 volunteers. Key functions and programming include: childcare, community development, health care, newcomers, seniors, food, and recreation. We serve over 20,000 unique participants and manage an organizational budget of almost $14M.   

    Scope of Work & Key Activities 

    The consultant will have to lead a comprehensive review of CNH’s technology infrastructure, practices, and systems, with the goal of supporting a coordinated, efficient, and future-ready IT environment that reflects the scale and diversity of CNH’s programs and services. The work will prioritize functionality, staff support, and practical improvements, while also identifying long-term strategic needs. The consultant will work collaboratively with staff across departments and leadership to ensure findings and recommendations are aligned with CNH’s operations and values.  

    Key Activities & Deliverables

    1. Organizational IT Assessment 

    • Conduct a full review of CNH’s current technology infrastructure and systems, including hardware, software, and network configuration. 
    • Review current IT-related procedures, policies, and practices (e.g., onboarding/offboarding with respect to IT, data storage, internal communication). 
    • Evaluate the use and effectiveness of key platforms such as Microsoft 365, point-of-sale (POS), accounting (SAGE), payroll systems (Rise People), health centre Cortico and EMR (Oscar), pharmacy software (WinRx) 

    2. Staff and Stakeholder Engagement 

    • Engage CNH staff and key participants to understand their IT-related needs, pain points, and suggestions for improvement. 
    • Gather input on current training gaps and technology adoption challenges. 

    3. Digital Tools and Data Systems 

    • Assess current data collection methods and systems (e.g., POS-based data tracking and reporting), with the aim of improving data quality and integration. 
    • Provide recommendations for a more comprehensive and efficient system for collecting participant data and program outcomes. 

    4. Systems and Process Improvement 

    • Provide practical recommendations for improving processes such as onboarding/offboarding, internal communication, and file management (on both server and cloud (Teams/OneDrive/SharePoint). 
    • Recommend improvements to phone, payment systems and communication systems to better support a hybrid, multi-site work environment. 
    • Engage with health centre’s clinical and administrative teams to evaluate EMR usage and propose enhancements in areas such as eForms, documentation templates, and process workflows 

    5. Cybersecurity and Digital Safety 

    • Conduct a general review of digital safety practices and cybersecurity risks. 
    • Identify any high-priority vulnerabilities and recommend basic protections to ensure data privacy and system integrity. 
    • Evaluate IT systems for adherence to HIPAA privacy and security policies and propose enhancements to ensure full compliance. 

    6. Training, Documentation, and Knowledge Transfer 

    • Develop or recommend staff training resources on Microsoft 365 and any new systems or tools. 
    • Produce user manuals or guidelines for standard IT procedures (e.g., email best practices, file management, security protocols). 
    • Develop preventative maintenance strategies for health centre and pharmacy equipment and contribute to efficient update procedure and break/fix support workflows 

    7. Final Report and Implementation Plan 

    Deliver a final report that includes: 

    • A summary of findings 
    • Short- and long-term recommendations 
    • Suggested implementation timelines 
    • Estimated costs for major changes (e.g., hardware/software upgrades, new systems) 
    • Present the report to CNH leadership and offer a debrief session to discuss the plan and next steps. 

    Budget & Timeline 

    The total project budget is $100,000 CAD, with approximately $80,000 CAD allocated for the core contract. Remaining funds are reserved for additional work or contingencies. 

    Work is expected to begin as soon as possible. Timeline and work hours can be included in the proposal. Preferred project duration would be approximately 6-10 months. 

    Proposal Requirements 

    Proposals must include: 

    • Consultant qualifications and relevant experience; Experience in non-profit, community health, or medical sectors is considered an asset 
    • A work plan detailing approach, timelines, and engagement strategy 
    • Description of deliverables and estimated costs 
    • Expected compensation 
    • At least two references from previous work or contracts
