    JOB OVERVIEW
    We are seeking a Registered Clinical Counsellor (RCC) to join Collingwood Neighbourhood House’s RISE Community Health Centre team.  The RCC will work with a diverse range of clients, including youth, children, couples and families to support their psychosocial needs through the provision of trauma informed one-on-one clinical counselling, family counseling and group therapy.  The RCC also supports mental health initiatives for the broader RC community in coordination with partner organizations and other departments of Collingwood Neighbourhood House.  The RCC coordinates client care by working collaboratively as part of an interdisciplinary team including other RCC’s, Nurse Practitioners, Physicians, dietitians, social workers, community health workers, medical office assistants, physiotherapists, pharmacists, nurses, occupational therapists and partners. The RCC communicates with the team by regularly participating in daily team huddles, monthly RCC meetings and RISE CHC staff meeting, and bi-annual CNH all staff meetings.  We value relational fit, clinical judgement, and lived or community experience, alongside professional credentials.

    Qualifications & Experience

    • Master’s degree in counselling from a recognized program, which includes a clinical supervised practicum
    • Registered and in good standing with the BC Association of Clinical Counsellors, or registration as an Expressive Arts Therapist or Art Therapist with their appropriate regulatory authority
    • Two (2) or more years of experience working in a low-barrier community setting counselling clients with complex and intersecting needs
    • Experience with single session counselling appointments for resourcing, risk assessment and relevant interventions
    • Experience applying strategies or practices to address social determinants of health and health inequity
    • Experience developing group curriculum for mental health programs
    • Experience utilizing a patient/client and family-centered recovery model in mental health
    • Experience, ability and comfort working with effects of trauma, attachment and developmental issues
    • Experience working with clients that have English language barriers and willingness to utilizing telephone interpretation as needed
    • Experience and willingness to provide services through outreach including people’s homes, community settings or virtually through phone or video call as required

    • Strong understanding of trauma-informed practice and the impacts of systemic and structural barriers
    • Ability to work flexibly and compassionately within the realities and limitations of health care and other intersecting systems
    • Demonstrated ability to assess patients/clients at risk and knowledge of appropriate legislation acts (e.g., Health Care Consent Act, Mental Health Act, Adult Guardianship Act, Continuing Care Act, Freedom of Information and Protection of Privacy Act) as it relates to the rights and obligations of patient/clients and staff
    • Proficiency in care planning, supportive counseling, crisis intervention, and care coordination
    • Proficiency in utilizing trauma-informed practice and strength-resiliency based approach
    • Proficiency in utilizing best practices in current assessment and psychotherapy for working with individuals, couples, groups, and families
    • Working knowledge of DSM (Diagnostic and Statistical Manual of Mental Disorders)
    • Demonstrated ability to work independently and collaboratively as a member of a diverse interdisciplinary team and a commitment to being a “team player”
    • Strong knowledge and experience of suicide risk assessment and interventions
    • Strong organizational skills, ability to establish workload priorities and good attention to detail
    • Willingness and ability to provide counselling in-person, by telephone and by video conferencing

        JOB OVERVIEW

        The light housekeeping and companionship program provides affordable, regularly scheduled cleaning services to seniors in the Renfrew-Collingwood community and offers a friendly visit with each appointment to socially isolated clients to increase connection and offset the adverse effects of loneliness. We are looking for a casual, on-call housekeeper/friendly visitor to help fill the growing demand for light housekeeping and social connections.

        SKILLS, QUALIFICATIONS & EXPERIENCE

        • Good communication, interpersonal and problem-solving skills
        • Patience and care for seniors’ needs
        • Experience with general housecleaning
        • Ability to use digital devices to update calendar and fill out timesheets
        • Speaks English and Cantonese fluently (Mandarin or other additional languages would be an asset)
        • Excellent time management skills
        • Proper work documents if the housekeeper is not a permanent resident or citizen such as an open work permit
        • Valid BC driver’s licence and access to a vehicle would be an asset

        RESPONSIBILITIES

        Housekeeper expectations include:

        • Communication:
          • Communicate with seniors, their family members, and the Housekeeping Program Manager in a respectful manner at all times
          • Communicate with seniors and/or family members to schedule service appointments for a mutually agreed time and dates
          • Continually update their digital calendar to document appointments (ability to use digital devices is required)
          • Communicate any changes to the client’s well-being or living situation to the Housekeeping Program Manager in a timely manner
          • Contact the Housekeeping Program Manager as soon as possible for any issues, disagreements, or problems
        • Health and wellbeing:
          • The housekeeper will conduct a self-screening for Covid-19 or any other illness such as cold or flu prior to entering a client’s home
          • The housekeeper will wear a mask when in the client’s home when appropriate and practice diligent hand hygiene
          • The housekeeper will follow emergent CNH policies as they relate to Covid-19 which are based on provincial recommendations

        • Social Visiting:
          • Enjoy getting to know your clients by spending time with them over a cup of coffee and sharing a treat

        JOB OVERVIEW
        We are seeking a Licensed Practical Nurse (LPN) to join the RISE CHC team. They will work as part of an interdisciplinary team with Nurse Practitioners, Family Physicians, Community Health Workers, Nurses, Allied Health, Medical Office Assistants and more. This role includes a focus on supporting our intake, referral, workflow and inventory management, in addition to a clinical LPN focus. The Licensed Practical Nurse will report to the RISE CHC Manager.

        The Licensed Practical Nurse is responsible for:

        • Providing high quality primary care in alignment with RISE CHC values and mandate
        • Practicing within the context of a client and family centered care model and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for Licensed Practical Nurses and within RISE polices, guidelines and clinical practice documents.
        • Providing primary care, health promotion education and outreach nursing including case management, wound care, administration of medications and immunizations, phlebotomy, mental health and addiction supportive care, chronic and infectious disease management.
        • Working with and supporting community partners through partnership, referrals and health related education and advocacy
        • Assisting in coordinating RISE CHC referral and intake process
        • Ordering and managing medical supplies and inventory.

        Qualifications

        • Graduation from an approved School of Nursing with current practicing registration as an LPN with the
          British Columbia College of Nurses & Midwives (BCCNM)
        • Two (2) years recent related nursing experience including one (1) year in an administrative capacity, or
          an equivalent combination of education, training and experience.

The Primary Care Supervisor will report to the RISE CHC Clinic Coordinator.

        The Primary Care Supervisor is responsible for:

        • Creating and implementation of clinical workflows, with input and support from the team
        • Supporting interdisciplinary staff with daily clinical workflow and service provision by acting as a clinical resource, supporting with client service navigation, creating and ensuring safe work practices, and reviewing and adjusting the CHC schedule to ensure client needs are met and clinician workload is balanced
        • Providing and/or coordinating clinical education and orientation activities with staff, including facilitating groups, coaching, conducting formal/informal presentations and demonstrations
        • Working with and supporting community partners through partnership, referrals and health related education and advocacy
        • Assisting in triaging RISE CHC referral and intake process
        • Assisting the Clinic Coordinator and Clinical Lead in the development, updating and implementation of RISE CHC policies and procedures
        • Assisting with recruiting, hiring, and retaining staff

        • Graduation from an approved School of Nursing with current practicing registration as a registered nurse (RN) with the British Columbia College of Nurses & Midwives (BCCNM)
        • Three (3) years recent related nursing experience including two (2) years in a leadership, or an equivalent combination of education, training and experience

        • Experience promoting collaboration, capacity building, teamwork, and communication within a diverse interdisciplinary team
        • Experience in overseeing clinic workflow and scheduling
        • Experience in arranging and providing in-services education/training for staff i.e. skills/procedures
        • Experience in providing culturally safe and trauma informed care
        • Experience in assessing and streamlining primary care service delivery i.e. Triage, Outreach, etc.
        • Experience in managing medical supplies and inventory
        • Experience in collecting blood samples
        • Proficient in using electronic medical records, word processing, internet and email software
        • Physical ability to perform the duties of the position
        • Current CPR certification

        • Demonstrated ability to triage clients and provide treatment planning, supportive counseling, crisis intervention, and case coordination/management
        • Able to communicate effectively, both orally and in writing, one-on-one and in group settings
        • Able to effectively employ conflict resolution and reconciliation techniques and approaches
        • Broad knowledge of crisis intervention and counseling skills
        • Broad knowledge of mental health and wellness, substance use and treatment, including harm reduction approaches
        • Broad knowledge of local community health and social service resources and referral options

        ASSETS

        • Fluency in identified non-official languages based on neighborhood demographics
        • Additional nursing courses, certifications, or experience in: immunization, foot care, PAP, addictions care, wound care, contraceptive management, and sexually transmitted infections
        • Experience working with medical interpreters
        • Training or experience in motivational interviewing techniques or Brief Action Planning
        • Advanced experience working with OSCAR or equivalent electronic medical record

        JOB DETAILS

        • Schedule: Negotiable within the parameters of 35-37.5 hours per week, Monday to Friday or Tuesday to Saturday during operating hours
        • Remuneration:
          o Salary range: $49.20-63.47/hr (based on years of experience in an RN Level 4 role)
          o Extended health and dental benefits, and multi-sector pension plan
          o Annual reimbursement for: British Columbia College of Nurses and Midwives licence renewal fees and CNPS liability protection fees
          o Paid vacation starting at 4 weeks per year (based on 35hrs/week) plus paid sick days
        • Location: RISE Community Health Centre, 5198 Joyce Street, Vancouver and community settings in Renfrew-Collingwood. Up to 1 day per week work from home (after probationary period)
        • Expected Start Date: ASAP
