Program Description:
The Social Prescribing Program aims to support socially isolated, low income, moderately frail, older adults and seniors, to reach their health and wellness goals through appropriate referrals and advocacy to community resources.
Day(s) and Time: Two (2) hours per week. Ongoing basis weekly. Hours between 8am to 4pm TBD between senior and volunteer
Location: In senior’s home near Nanaimo/Hastings in Vancouver
Duties:
- Visit with the senior in their home
- Walk/bus with the senior to the grocery store and bank acting as moral support
- No physical support is needed or permitted
Requirements:
- Criminal Records Check Clearance
- Two character reference checks needed (past volunteer supervisor, employer, teacher/professor)
- Vaccine records required
- Signing of CNH onboarding documents
- Must be comfortable being alone in a home with a senior without staff supervision
