Collingwood Neighbourhood House (CNH) is a not-for-profit Society serving the Renfrew-Collingwood (RC) neighbourhood of East Vancouver. CNH’s mandate is to promote the well-being of the Renfrew-Collingwood community by providing leadership and working collaboratively with individuals, families, agencies and other groups to develop and support inclusive, innovative, sustainable initiatives and services that respond to the community’s social, educational, economic, health, cultural and recreational needs.
RISE Community Health Centre (CHC) is a department of CNH, and is funded by the Ministry of Health. RISE CHC supports the health and well-being of Renfrew-Collingwood residents by offering community-based primary care and health promotion services. Populations of focus include Newcomers, LGBTQ2S+, Indigenous, Isolated Seniors, Youth, people experiencing Homelessness, those working in the Sex Trade, and those experiencing Mental Health and Substance Use issues. Services are delivered by a diverse interdisciplinary team, working in partnership with other Collingwood Neighbourhood House departments and partnering agencies.

JOB OVERVIEW
We are seeking an Occupational Therapist (OT) to join our interdisciplinary team. The OT role focuses on supporting clients to overcome barriers related to injury, illness, disability or social and environmental challenges, to enable participation in meaningful everyday activities (occupations). We anticipate the OT will spend at least half of their time in community settings. The OT will receive internal referrals from RISE CHC clinicians for support or consultation regarding:
Health promotion
- Self-management
- Chronic disease management
- Joint protection
- Pain management
- Skin integrity management
- Fatigue management
- ADHD & Sleep disorder
- Mental health and substance use management
- Cognitive impairment
- Functional mobility and falls and injury prevention
- Community integration and transportation options
- Community resources and social support services
The RISE OT will also be involved in facilitating or co-facilitating therapeutic or health promotion related groups. - SKILLS AND QUALIFICATIONS
- Graduation from a recognized occupational therapy program.
- Current registration with the College of Occupational Therapists of British Columbia (COTBC)
- Two (2) years’ recent, relevant patient care experience, ideally in a team-based primary care setting or equivalent combination of education, training and experience
- Proficient in providing culturally safe, trauma informed care to socially complex clients relevant to RISE CHC’s priority populations
- Demonstrated ability to assist individuals to achieve their goals and maintain or rebuild their independence, to participate in everyday life
- Demonstrated ability to optimize participation in self-care (ADL/IADL), leisure, work, social and community activities
- Demonstrated ability to assess the whole person including psychological, physical, emotional, cognitive and social functions, and the environmental and social context
- Demonstrated ability to provide consultation, education and coaching to prevent exacerbation of existing conditions or the development of new chronic conditions
- Demonstrated ability to provide and coordinate treatment and care planning focused on client goals
- Demonstrated ability to provide resources and link to community services and social support groups
- Demonstrated ability to refer the client to the right service provider, including other RISE CHC team members; and/or other public and private service providers
- Demonstrated ability to collaboratively plan and co-facilitate therapeutic or health promotion groups
- Demonstrates a high level of clinical skill and competence working with socially complex clients

- Demonstrated ability to work independently and collaboratively as a member of a diverse interdisciplinary team and a commitment to being a “team player

- Strong organizational skills, ability to establish workload priorities and good attention to detail
- Ability to utilize Electronic medical records, word processing, Internet and email software
- Experience applying strategies or practices to address social determinants of health and health inequity
- Experience and willingness to provide outreach in community settings and in people’s homes
- Ability to build relationship with community partners
- Ability to deal effectively with conflict situations, manage challenging behavior, and use non-violent crisis intervention and behavioral management techniques
- Physical ability to perform the duties of the position
- Must be fully vaccinated against COVID-19

- Salary: $42.56 – 52.82/hour

- Annual reimbursement for: professional dues (prorated to FTE)
- Paid vacation: starting at 3 weeks per year (prorated for FTE) plus paid sick days
Schedule: 22.5 hours per week (0.6 FTE), Tuesday 9 – 5:00pm, Friday 9-5:00pm, Saturday 9-5:00pm, half-hour unpaid lunch break
